Add readme and changelog

The readme is displayed on the extension page when the user installs the extension, so it should contain helpful information for getting started. I changed the display name to "SOI Code" and improved the short description.

Additionally, I added important changes since the last release to the changelog. Before releasing, "Unreleased" should be replaced by the version number and date.
